Clozapine. What is the Preparation?


Clozapine

Clozapine is a synthetic antipsychotic, used to treat various mental and sleep disorders, including children.

The preparation treats schizophrenia, resistant to all other drugs. The drug has inhibitory action on the central nervous system and its normal doses do not cause soporific effect, it also has sedative effect on the central nervous system.

Clozapine improves status of patient with such symptoms as: exaltation, delusion, hallucinations, suspicion, apathy, disorientation, emotional decline, lack of satisfaction.

Historically Clozapine was the first med of the class of so-called “atypical antipsychotics”, in other words, – antipsychotics, the unique feature of which is slight chance of extra-pyramidal adverse events, better tolerability and less influence on the lactation hormone’s secretion.

In 1975 the manufacturer of Clozapine withdrew this drug from circulation due to the frequent adverse effects on the blood (granulocytopenia up to non-survivable agranulemia), but then at the end of 1980 the preparation was returned to the range of the medications, approved by the Food and Drug Administration (FDA).

The dosing schedule is set individually. A single oral dose is 50-200 mg. and 200-400 mg. per day.

Usually the treatment is started with the dose of 25-50 mg., then this dose should be gradually increased to 25-50 mg. per day up to 200-300 mg. per day for 7-14 days.

The daily dose can be used as a single dose at bedtime or 2-3 times/day after meal. If there is cessation of therapy the dose should be gradually titrated down for 1-2 weeks.

If necessary, Clozapine can be inserted intramuscularly

The maximum per os dose is 600 mg/day.

90 consecutive patients with sexual problems


Spanish researchers have uncovered clear links between erectile dysfunction (ED) and peripheral neuropathy, according to a paper in the December issue of the urology journal BJUI.

“Up to now the impact of damaged nerves in the peripheral nervous system on ED has been underestimated” says lead author Dr Consuelo Valles-Antuña, from the Department of Neurophysiology at the Hospital Universitario Central de Asturias in Oviedo.

“However our study of 90 patients shows that men with more severe symptoms of peripheral neuropathy, which can be caused by disease, trauma or illness, had greater self-reported ED and required more aggressive treatment.

“Our findings underline the importance of clinicians carrying out neurophysiological tests on patients with ED, particularly in the pelvic area.”

The research team, which included experts on both neurophysiology and urology, studied 90 consecutive patients with sexual problems recruited from the hospital’s Department of Andrology.

ED was diagnosed using the five-item version of the International Index of Erectile Dysfunction (IIEF-5) and the occurrence of peripheral neuropathy was predicted using the Neuropathy Symptom Score.

A range of neurophysiology tests were carried out to assess the presence of large and small fibre peripheral neuropathy.

The researchers found that:

The average age of the men in the study was 54 years of age. Ten per cent were under 40 and only two per cent were over 70.

No significant correlation between IIEF-5 scores and increasing age was found. In fact, younger patients had lower (worse) IIEF-5 scores, which could be due to higher expectations or a higher number of organic risk factors.

Just under a third of the patients (30 per cent) had cardiovascular disease, 16 per cent had neurogenic risk factors (relating to the nerves or nervous system) 16 per cent had diabetes and 11 per cent had no risk factors. Just over seven per cent had been diagnosed with mental health issues.

Patients with more severe symptoms of peripheral neuropathy showed lower (worse) IIEF-5 scores and required more aggressive therapies.

Neurophysiological exploration confirmed that just under 69 per cent of patients had neurological pathology. Of these, 61 per cent had some type of peripheral neuropathy and eight per cent had myelopathy problems with their spinal chord.

Just under 38 per cent of the patients had polyneuropathy, which occurs when a number of the peripheral nerves throughout the body malfunction simultaneously. Of these nine per cent had small fibre neuropathy, damage to the small unmyelinated peripheral nerve fibres, and just over 14 per cent had pudendal neuropathy, affecting the somatic nerve in the pelvic region.

The findings of the sympathetic skin response tests underlined the importance of checking nerve problems in the pelvic area, as response alterations were much more common in the penis than hand or foot.

No association between neurophysiological diagnosis and IIEF-5 scores was detected, but a statistical association was found between neuropathy and the Neuropathy Symptom Scores.

“To our knowledge, this is the first study to assess the whole peripheral nerve fibre spectrum in a non-selected group of patients with erectile dysfunction” says Dr Valles-Antuña.

“By combining feedback from patients with a series of neurophysiological tests we have been able to demonstrate that peripheral neuropathy is highly prevalent in patients with ED.

“However, we found that the IIEF-5 scores and Neuropathy Symptom Scores were not reliable tools for detecting neurological alterations in patients with ED.

“Our findings suggest that clinicians should carry out neurophysiological tests on patients with ED, paying particular attention to the pelvic area.”

Sales of Viagra are highest in the West Country city.


A team at the Medical College of Georgia in the US found that the toxins in the spider’s venom were an effective treatment for impotence in rats, suggesting it could be used to help give men and women a boost in the bedroom.

Study author Dr Kenia Nunes said: We found the toxin responsible and performed experiments using hypertensive rats which have severe erectile dysfunction. The toxin was able to normalise the erectile function in these animals.

This is good because we know that some patients don’t respond to the conventional therapy, she added.

This could be an optional treatment for them.

A bite from the spider could also have some other, nastier, side effects, including loss of muscle control, severe pain, difficulty breathing and oxygen deprivation.

However, the scientists did manage to help the rats achieve side-effect-free erections.

The wandering spider, which is thought to be the most venomous spider on Earth, is also known as the armed spider, banana spider or Phoneutria nigriventer, and is found in South and Central America.

A previous survey by supermarket giant Tesco highlighted that men in Bristol may be most in need of a bite from the spider, after it emerged that sales of Viagra are highest in the West Country city.

The study surveyed American singles


According to a new study by the National Campaign to Prevent Teen and Unplanned Pregnancy, many young American men exhibit attitudes toward contraception that could best be described as “magical.” The study [PDF] surveyed American singles ages 18–29 about their perceptions about and use of contraception. Twenty-eight percent of young men think that wearing two condoms at a time is more effective than just one. Twenty-five percent think that women can prevent pregnancy by douching after sex. Eighteen percent believe that they can reduce the chance of pregnancy by doing it standing up.

For the most part, men lagged behind women on the pregnancy prevention front. And when the study dipped into the realm of “female” forms of birth control, the gender divide intensified. In the study, 29 percent of men and 32 percent of women reported that they know “little or nothing about condoms.” When asked to rate their knowledge of birth control pills, 78 percent of men reported to be clueless, compared to 45 percent of women.

In addition to staging teach-ins, women are also responsible for shouldering the physical, emotional, and financial responsibilities for pregnancy prevention. Pap smears, STI tests, and gynecological sessions about their contraceptive options—that’s just the tip of it for the sexually active woman. In order to keep their birth control subscription fresh, they have to repeat that process every year. Their male sex partners are under no such requirements. As Salon noted last year, women have 11 methods of contraception from which to choose; men have two—condom and vasectomy. And even if men did have additional reliable birth control options, many women wouldn’t trust them to use them correctly. That sounds pretty awful. I thought it, uh, somehow killed fertility with like chemicals and stuff,” he says. The ring, however, strikes Seestedt as a superior option. “I thought that was pretty fine, because, from what I understood, it was kind of a low-cost way of doing it, and it wasn’t really…I like it because chemical pills and stuff are kind of weird, [but the ring] was kind of placed inside, and…you know what I mean? It just kind of did its thing, you know?”

To some, the male indifference to birth control can be attributed to a juvenile disregard for all things related to the place in which the vaginal ring “does its thing.” We live in a country where heterosexual heartthrob Robert Pattinson feels comfortable announcing to Details magazine, “I really hate vaginas. Two droplets of blood


Two droplets of blood, technically known as haemolymph, have been preserved in the amber which also contains the spider — Filistatidae — a family commonly found in South America and the Caribbean.

The droplets are the first identified examples of spider blood ever found in an amber fossil. It is possible the blood could be used to extract DNA.

The fossil, which is 4cm long and 2cm wide, was discovered in the Dominican Republic and dates back to the Miocene period.

Palaeontologist Dr David Penney, of the School of Earth, Atmospheric and Environmental Sciences, has now used the blood droplets to trace how, when, and where the spider died all those years ago.

David, said: “It’s amazing to think that a single piece of amber with a single spider in it can open up window into what was going on 20 million years ago.

“By analysing the position of the spider’s body in relation to the droplets of blood in the amber we are able to determine how it died, which direction it was travelling in and even how fast it was moving.”

In the latest issue of the journal Palaeontology (2005, vol. 48, part 5) David describes how the spider died. He believes the spider was climbing up a tree when it was struck head-on by a sudden strong flow of resin. The spider then became engulfed in the resin and died.

He argues that the shape and position of the blood droplets reveals which direction the spider was travelling in. It also reveals which of the spider’s legs broke first.

David discovered the fossil in 2003 during a visit to the Museo del Ambar Dominicano in Puerto Plata, Dominican Republic. His research initially focused on the spider which he identified as an entirely new species of spider. On his return to the UK, further research revealed the droplets of blood and the information the fossil contained.

Children in America without dental care


One in five children in America goes without dental care each year, according to a new national study released by the Pew Center on the States.

The study also found that two-thirds of the 50 states have ineffective policies to ensure proper access to care. Utah received a “D” grade for its policies for providing dental health care to low-income families, meaning the Beehive State met three of eight policy benchmarks used by researchers such as school-based dental sealant programs, fluoridated public water supplies and Medicaid payments.

Other states that received “D” grades were Alabama, Indiana, Mississippi, Montana, Nevada and the District of Columbia. Meanwhile, Arkansas, Delaware, Florida, Hawaii, Louisiana, New Jersey, Pennsylvania, West Virginia and Wyoming received “F’s” for their efforts.

The release of the study, which coincides with National Children’s Dental Health Month, sheds light on the need for improved dental care for needy children.

Aside from the misery of tooth decay and possible infection, neglecting children’s dental care can have lifetime consequences, according to the report.

For example, 42 percent of incoming Army recruits had at least one dental condition that needed to be treated before they could be deployed, according to a 2000 study of the armed forces. More than 15 percent of recruits had four or more teeth in need of urgent repair.

In recent years, child advocates have termed tooth decay among Utah children as an “epidemic.” The Utah School Nurse Association has identified dental disease as the biggest health problem among elementary school-age children.

Beyond the personal costs, poor dental care among children is costly to society. The Pew study estimates that annual spending for dental care in the United States will increase 58 percent between 2009 and 2018. One-third of the money spent goes to services for children.

One can argue about methodology of the survey, but the issue deserves attention. Yes, dental care is largely a parental responsibility — good dental hygiene, regular check-ups, taking steps to prevent baby bottle tooth decay, wearing mouth protectors and knowing what to do in the event of a dental emergency.

But government policies that preclude children from obtaining needed dental services that not only enhance dental health, but promote physical health and children’s ability to learn, need to be reviewed by all states.
